<li>Assist individuals with arrangements for transportation via agency van and/or ACCESS. </li>
<li>Attend individuals' ISP meetings. </li>
<li>Schedule and accompany individuals to appropriate appointments. </li>
<li>Ensure that immunizations, written recommendations of providers, and all medical follow-ups are completed as required.</li>
<li>Collaborate with Residential Team to ensure that regulatory requirements are in compliance.</li>
<li>Coordinate community activities with/for individuals and ensure appropriate staff support is in place.</li>
<li>Manage vacant direct care shifts and potentially fill in for emergency situations or other circumstances. </li>
<li>Work with Quality Coordinator to ensure compliance with regulations.</li>
<li>Supervise and/or report critical incidents within 24 hours and provide all necessary information.</li>
<li>Acquire 24 hours of relevant training, including required trainings, during the designated cycle.</li>
<li>Other duties as assigned.</li>
