SUMMARY:A key component of this job is to assist clients in their physical, social, emotional and daily life skills development. This increases their independence and allows them to function appropriately in the community. Services provided include crisis intervention, behavior management, and life skills training, whether on a one-to-one basis or in a group environment. Plans, develops, implements and evaluates recreational, social and/or educational activities for clients served in the program. Proof of COVID-19 vaccinations is required.
 
DUTIES &amp; RESPONSIBILITIES:

<li>Aids clients in their own homes/program and in the community to allow them to function more safely and independently. </li>
<li>Facilitates physical, recreational, social and educational activities. </li>
<li>Provides clients with a positive role model.</li>
<li>Assists clients with personal hygiene and grooming i.e. bathing, oral hygiene, and toileting.</li>
<li>Monitors clients’ needs, progress and well-being and assists case management team in evaluating the effectiveness of planned counseling and support</li>
<li>Assists case management team in identifying potential problems. Provides input to counselors and other professionals with regard to the development of client program plans</li>
<li>Teach daily living skills such as budgeting, shopping, meal preparation, etc.</li>
<li>Assist in the monitoring of clients’ physical and mental health</li>
<li>Work with other professionals involved with the client, acting as a learning support assistant</li>
<li>Document all contacts and outcomes in the individual case file</li>
<li>Advocates on behalf of clients to secure social services entitlements</li>
<li>Assist clients with applications for entitlements</li>
<li>Maintains contact with all necessary collaterals and attends appropriate meetings</li>
<li>Maintains on-going communication with Coordinator/Case Managers on all aspects of the work</li>
<li>Some evening and weekend work will be required. </li>
<li>Other duties as required or assigned by supervisor</li>

EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTs:
A minimum of an Associate’s degree or at least 2 years of experience working with substance abuse, mental health, homeless and re-entry population. Successful applicant must have strong written and verbal communication skills; must be able to provide good customer service, both in person and by telephone; must possess good organizational skills and have a high attention to detail. Must have the ability to handle multiple tasks, prioritize work assignments, and maintain confidentiality. Bi-lingual Spanish a plus

Provide reassessments on a regular basis, as requested by Resident Care Director or Generations Care Director. </li>
<li>Conduct interviews and observations on a continual basis of the residents to alert the Resident Care Director, Generations Care Director and others of changes in the physical or emotional health of the resident. Participate in weekly service plan meetings, as requested.</li>
<li>As directed by the Resident Care Director or Generations Care Director, complete all required data entry into the Eldermark to ensure maintenance of resident care information. We Need: InReach needs a DSP (Independent Contractor) for a male Individual with Developmental Disabilities in the Charlotte area, 28226. Staff will provide Community Living and Support Services for about 17 hours a week.
Schedule:
M-F: 4:30 pm 
Sat/Sun: Anytime between 9a-9p! Pay Rate:
CLS: $20.35/hr
 
What You'll Do:
Instruction and assistance in home living environments.
Help individual identify and acquire valued social roles.
Facilitate community integration.
Help individual develop healthy social habits.
Dispense medication and monitoring dosages.
Transport and accompany individual to and from day programs, work sites, and community activities.
All InReach applicants must have a high school diploma (or GED), a valid driver's license, and successfully pass a drug screen and background check. One to two years of experience with the I/DD population preferred.
*** This DSP position is an Independent Contractor position. Independent Contractors receive a 1099 tax form and use a third-party Contract Management vendor for their direct deposits. Additional fees required for Contractors include paying for state-required training, Occupational Accident Insurance, and administrative fees for the Contract Management vendor.
