Position Summary:

Primary Responsibilities:
Resident Services
1. Provide quality care and assistance to residents in accordance with the Oaks Senior Living philosophy of Person-Centered Lifestyle and in accordance with resident rights.
2. Assist residents with the activities of daily living according to individual resident preferences.
3. Protect the confidentiality of personal and medical information of residents and coworkers.

POSITION SPECIFIC DUTIES AND RESPONSIBILITIES:
ERSEA
<ul>
<li>Develop, maintain, and manage system to track the eligibility, recruitment, selection, enrollment, and attendance of children to comply with federal and state program regulations. </li>
<li>Annually create the criteria for the selection and enrollment of children assuring responsiveness to community assessment and reflecting data on dual language learners, homeless children, children in foster care, and children with disabilities. </li>
<li>Verify income and eligibility qualifications of children and families, and ensure applications are complete and data is accurately entered into the Child Plus database. </li>
<li>Create and maintain record keeping and reporting policies for waitlists, enrollment, attendance, timelines, schedules, and procedures in accordance with designated state and federal program requirements. </li>
<li>Ensure ongoing monitoring, tracking, follow-up, and analysis of enrollment and attendance data, and produce regular reports for management meetings. </li>
<li>Assume lead role in being proactive in addressing any possible child suspensions and in the prohibition of expulsion and collaborate with the Mental Health Services. </li>
<li>Maintain enrollment forms, ensuring information is current, correct, and disseminated to all necessary staff to meet program requirements. </li>
<li>Direct recruitment activities in response to enrollment data and waitlist data. </li>
<li>Supervise ERSEA department staff, monitoring performance, providing evaluations, facilitating goal setting, and staff development. </li>
<li>Ensure full program enrollment and maintenance of a sufficient waitlist. </li>
<li>Compile and submit Program Information Report (PIR) data periodically. </li>

Family Service/Community Partnerships

<li>Assume the lead role in creating a system for the development and implementation of Family and Community Engagement Services. </li>
<li>Assume the lead role in the development, implementation, record keeping, and reporting of the Family Partnership Agreement and assure its alignment to the Parent Family Community Engagement Framework. </li>

PHYSICAL RESPONSIBILITIES  
Ability to see at normal distance, hear normal conversations and sounds and use hands and fingers to fill out required forms. Ability to lift infants and toddlers weighing up to forty (40) pounds, twenty (20) pounds overhead and forty (40) pounds from waist to shoulder; occasional lifting of fifty (50) pounds is required and must be able to push/pull up to fifty pounds horizontally. Ability to sit on the floor, bend at the waist, kneel and/or stoop 75% of the time.

Our Mainstay United program offers a range of residential, behavioral, and independent living skills support, within a rehabilitative model, for individuals with intellectual disabilities. The program specializes in helping people who have a history of problematic sexual behavior by incorporating clinically centered practices into everyday living. Mainstay United uses trauma-informed and person-centered positive approaches to address risk-reduction for the community and to assist individuals in gaining independence
Job Summary:
As a Direct Support Professional, you will play a crucial role in enhancing the quality of life for individuals with disabilities. Your responsibilities will include assisting clients with daily living activities, promoting independence, and providing emotional support and companionship.
Key Responsibilities:
· Support clients in daily living activities, including personal care, meal preparation, and household management.
· Foster a positive and respectful relationship with clients while promoting their independence and personal choices.
· Assist clients in developing skills for everyday living and participate in social and recreational activities.
· Document client progress and maintain accurate records of care and support provided.
· Communicate effectively with clients, families, and other team members to ensure coordinated care.
 
Qualifications:
· High school diploma or equivalent; further education in a related field is a plus.
· Previous experience in direct support, caregiving, or a related field preferred.
· Compassionate and patient demeanor with a genuine desire to help others.
· Strong communication and interpersonal skills.
· Ability to work independently and as part of a team.
· Valid driver's license and reliable transportation.
· CPR and First Aid certification (or willing to obtain).
Ability to participate in background check and driver's record check.

JOB SUMMARY:
A Personal Care Attendant generally is responsible for light cleaning, cooking, running errands, and doing laundry, as well as assisting clients with bathing, showering, grooming, and other personal hygiene tasks. They also engage clients in activities like reading, talking, and general companionship. 
 
QUALIFICATIONS:
<ul>
<li>High school diploma or equivalent</li>
<li>Must be able to pass a fingerprint/background check.</li>
<li>Valid state driver’s license </li>
</ul>
SKILLS/ABILITIES:
<ul>
<li>Assist clients with basic hygiene and grooming duties to promote adequate self-care.</li>
<li>Help clients dress, bathe, eat etc.</li>
<li>Ensure clients receive their medication on time (medication reminder)</li>
<li>Do routine housekeeping chores—including various cooking and cleaning duties—that the client is otherwise unable to do themselves.</li>
<li>Transport clients to appointments, errands, and other necessary off-site locations- when required.</li>
<li>Provide physical assistance—such as help getting in and out of bed or walking with a walker—to clients when required.</li>
<li>Maintain detailed records of the client’s activities and report any relevant changes to the supervisor.</li>
<li>Act quickly and responsibly in cases of emergency</li>
<li>Provide companionship and emotional support.</li>
