SUMMARY This position will be responsible for connecting with clients within targeted geographic communities to provide outreach, education and training on targeted subject matter; including substance abuse disorders, community resources, pre-arrest deflection, health insurance, etc. This position will serve as a public face of TASC in the designated areas and be responsible for direct client services and community partners relations and training.
ESSENTIAL DUTIES AND RESPONSIBILITIES Assist individuals in achieving service plan goals through direct services and coordination of behavior health, public agencies, and community resources.

<li>Make referrals to all essential needed direct services: Substance abuse disorder treatment, recovery support services, housing, etc.</li>

POSITION TITLE: Resident Care Associate
DEPARTMENT: Nursing
REPORTS TO: Resident Care Director
POSITION SUMMARY:
<ul>
<li>The essential functions of the job for the Resident Care Associate requires, and may be amended from time to time at the sole discretion of Viva Senior Living to include, but are not limited to the following:</li>
<li>Comply with standards of business conduct in accordance with federal, state, and local health and regulatory standards and guidelines, as applicable.</li>
<li>Must possess, at a minimum, a high school diploma or a GED </li>
<li>Must be able to work well with peers in a congenial work environment, and above all, maintain the utmost compassion and dignity to the residents and adhere to the Resident Bill of Rights.</li>
<li>In exigent circumstances, the employee may be subject to call back during emergency conditions (e.g., severe weather, evacuation, temporary staffing shortages, etc.).</li>
<li>Must adhere to Viva Senior Living’s attendance policies and follow the terms and conditions of employment as set forth in the Employee Handbook.</li>
<li>Must be able to read, write, speak, and understand English.</li>

RESPONSIBILITIES/ACCOUNTABILITIES
<ul>
<li>Provide direct care to residents and contribute to continuity of care and effective communication by reporting care provided and changes in the resident's condition to the Resident Care Director immediately. May participate in routine activities of resident’s care activities, including daily living.</li>
<li>Assisting residents with lifts, moves, and transports, using proper body mechanics or lifting devices for accident prevention.</li>
<li>Responds to inquiries relating to requests from residents, visitors, and other personnel promptly and courteously within given time frames and established policy.</li>
<li>Is knowledgeable of the individualized care plan for residents and provides support to the residents according to their care plan. Contributes to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents’ needs and preferences.</li>
<li>Communicates and interacts effectively and tactfully with the residents, visitors, families, peers, and supervisors promoting a homelike environment.</li>
<li>Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents’ rights.</li>
<li>Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials. Also providing care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas.</li>
<li>Arrive to work at the scheduled time.</li>
<li>Perform other related duties as required.</li>
<li>Ensures that residents and families receive the highest quality of service in a caring and compassionate atmosphere which recognizes the individuals’ needs and rights.</li>
<li>Report any occupational exposures to blood, body fluids, infectious materials, and or hazardous chemicals in accordance with facility policy.</li>
<li>Report work related injuries and illnesses immediately to your supervisor.</li>
<li>As a condition of employment, complete all assigned training and skills competency.</li>
<li>Participate in all life safety and emergency drills and trainings.</li>
<li>Fulfill responsibilities as assigned during implementation or activation of the facility’s emergency plan.</li>
<li>Assists with evacuation of residents in an emergency situation</li>
<li>Maintain confidentiality of Protected Health Information, including verbal, written and electronic communications.</li>
<li>Report non-compliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.</li>
<li>Protect residents from abuse and cooperate with all investigations.</li>
<li>Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator.</li>
<li>Report any retaliation or discrimination to the Human Resource Director and the Compliance Officer.</li>
<li>Fulfill individual responsibility to report any suspicion of a crime to law enforcement and the state agency.</li>
<li>Performs other duties as requested.</li>
