Job Description

The Resident Care Coordinator is responsible for providing personal assistance and support to individuals who need help with daily living activities due to age, disability, or illness, including tasks like bathing, dressing, eating, medication reminders, mobility assistance, and emotional support, often working in a home setting or care facility, while monitoring their health and communicating any changes to family members or medical professionals.

  • Personal care: Assisting with bathing, dressing, grooming, toileting, and other personal hygiene needs.
  • Mobility assistance: Helping clients get in and out of bed, chairs, and wheelchairs.
  • Meal preparation: Preparing meals according to dietary restrictions and assisting with eating.
  • Medication management: Reminding clients to take medication as prescribed, under supervision.
  • Health monitoring: Observing and reporting changes in physical and mental health, including vital signs.
  • Companionship: Providing emotional support, conversation, and engaging in recreational activities with the client.
  • Light housekeeping: Performing basic cleaning tasks related to the client's care.
  • Transportation assistance: Accompanying clients to medical appointments or errands.
  • Communication and documentation: Maintaining detailed records of client's care, reporting changes to family members and healthcare providers.
