Job Description:
The Day Habilitation Assistant supports individuals with developmental disabilities in achieving greater independence, community integration, and personal growth. This role provides direct assistance with daily living skills, implements individualized service plans (ISPs), documents progress, and helps ensure a safe, engaging, and supportive program environment.
. Provide and facilitate opportunities for community based recreational and educational activities for individuals supported.

Your Job responsibilities include:
Supervision and oversight of day habilitation programs; ensuring respectful treatment of participants and staff, advocacy, independence, and adherence to regulatory standards
- Daily coordination, oversight, implementation of direct care, documentation of individualized program plans, and overall day-to-day operations of the program site.
- Daily/weekly/ Monthly Calendar Planning for outings and activities volunteering etc.
- Provide direct personal care (e.g. toileting, transferring), medication administration, documentation, and transportation.
- Act as liaison with outside agencies, families, and service providers; ensure quality service delivery, safety, and active staff and individual engagement.
- Lead, guide, training staff and ensuring compliance with service plans, documentation; report any issues to QDDP and or Human Resources.
- Staff scheduling
- Support agency mission and values, comply with policies, and perform other duties as assigned.
- Emergency procedures, safety, record-keeping, confidentiality, physical assistance duties, crisis interventions, transportation assistance as needed
- Assisting with daily living skills and personal cares.
- Engaging with young adults – meeting them where they are at and mentoring.
- Assisting with coping and communication skills
- Collaborate with coworkers, supervisors, and interdisciplinary team members.
- Participate in staff meetings, training, and performance reviews.
- Serve as a positive role model for individuals, demonstrating respectful and professional behavior.
- Completing all documentation and training requirements
- Day-Hab billing
- Other Duties as assigned
Who are we looking for?
The idea candidate for the role:
· Valid Driver’s license and vehicle insurance
· 1 year Supervisory experience
· Two (2) years related experience, preferred, not required.
- Caregiving: 1 year (Preferred)
- Developmental disabilities: 1 year (Preferred)
· Willing to weekends when needed
·Pass a background check
· Able to work with pets (cats/dogs or both)
Schedule
- Full-time: Monday–Friday, approximately 7:45 AM – 4:30 PM
- Participate in the On-Call Rotation

- Pay range: $20-$22 per Hour DOE
