Hybrid Pediatric RN Care Manager
As a Hybrid Pediatric RN Care Manager, you'll use your passion for pediatric care to help patients and their families find the support and resources they need to thrive! With the flexibility of a hybrid work environment,no weekends, evenings or Holidays, you'll be able to find a work life balance while changing lives. Pay Details: $55,100 - $99,000 a year
- Annual Merit Increase
- Mileage Reimbursement
Shift: Monday – Friday | 8a-5p | No on call, weekends, evenings, or Holidays!
Daily Responsibilities For The Hybrid Pediatric RN Care Manager
- Coordinate and manage comprehensive care plans for medically fragile children (ages 0–21), including post-hospitalization assessments, follow-ups, and connection to community resources to improve outcomes and reduce readmissions
- Collaborate with families, providers, and specialists to address medical and social needs, ensure continuity of care, and complete documentation remotely
Required Qualifications For The Hybrid Pediatric RN Care Manager
- Active, unrestricted Florida state Registered Nurse (RN) license
- 2+ years of recent Pediatric experience as a RN
