This role empowers patients to take charge of their health through behavior change, self-monitoring, and collaboration with the interdisciplinary care team. Responsibilities
- Provide patient outreach via phone, video, and in-person coaching sessions.
- Support lifestyle changes including nutrition, exercise, stress management, and smoking cessation.
- Teach patients the use of self-monitoring tools such as blood pressure cuffs, glucose meters, and weight scales.
- Assist patients in setting realistic, individualized health goals and track progress over time.
- Document coaching interactions in the electronic medical record (EMR).
- Collaborate closely with nursing staff and care coordinators to ensure continuity of care.
- Performs other duties as assigned.
Qualifications
- High School Diploma or GED.
- Bachelor’s degree in Health Education, Nutrition, Exercise Science, Public Health, or a related field preferred.
- Two years of experience in health coaching, wellness, or chronic condition support.
- Certification as a Health Coach (CHC, NBC-HWC, or equivalent) preferred.
