Summary:

The Case Manager provides comprehensive case management services to clients, supporting their recovery journey and coordinating with clinical staff and external agencies. Responsibilities include assessment, planning, advocacy, and resource linkage to ensure clients receive the necessary support and services.

Essential Duties and Responsibilities:

  • Offer appropriate services to meet client needs.

  • Maintain accurate files and record keeping, which includes assessments, individual treatment plans, admissions, progress notes, discharge summaries, and discharge recommendations, to meet regulation requirements.

  • Conduct assessments to determine clients’ needs, strengths, and goals.

  • Develop and implement individualized service plans in collaboration with clients and clinical staff.

  • Provide ongoing case management, monitoring clients’ progress, and adjusting plans as needed.

  • Advocate for clients and link them to appropriate resources, services, and support systems.

  • Work closely with clinical staff to ensure coordinated care and comprehensive support for clients, to include counseling, education programming, family therapy, medical services, and discharge planning.

  • Communicate regularly with external agencies, service providers, and community resources.

  • Facilitate and participate in case conferences, team meetings, and educational presentations.

  • Maintain accurate and up-to-date client records, including assessments, service plans, progress notes, and reports.

  • Ensure compliance with all regulatory requirements and organizational policies.

  • Complete all required documentation and reports in a timely manner.

  • Provide crisis intervention support as needed, responding promptly to emergencies.

  • Collaborate with clinical staff to develop and implement crisis management plans.

  • Complete all required reports and paperwork in a timely manner.

  • Attend and participate in staff training, work teams, and meetings as directed.

  • Ensure client needs are met to the extent possible through effective case management.

  • Serves as a resource throughout the facility at all levels of care.

  • Follows necessary requirements as determined by the state in relations to licensure and/or certification.

  • Follow HIPAA and 42 CFR confidentiality requirements.

  • Strictly abide by all agency and program policies and procedures.

  • Other duties as assigned.

Supervisory:

This position does not oversee staff.

Education/Experience

  • Bachelor’s degree in social work, psychology, or a related field preferred.

  • Previous experience in case management, social services, or a related field.

  • Knowledge of community resources and service coordination.

  • Ability to manage multiple cases and prioritize tasks effectively.

Work Environment:

The work is performed in a typical office setting with minimal exposure to physical risks. Requires frequent interaction with staff, clients, and external visitors.Standard office hours with occasional need for extended or on-call hours including nights and weekends.

Physical Demands:

  • Ability to stand, walk, bend, and reach for extended periods.

  • Ability to thrive in a fast-paced, deadline-oriented work environment.
