Position: HIV Medical Case Manager
Salary Range: $45,000 – $49,500 per annum, with benefits
Status: Full-time
Responsibilities
A Medical Case Manager for HIV disease provides client-centered support to individuals living with HIV/AIDS by helping them access and maintain medical care, psychosocial services, and other necessary resources to achieve optimal health outcomes and quality of life. This role involves assessing needs, developing individualized care plans, coordinating services, and advocating for patients.
Key Responsibilities:
- Client Assessment and Care Planning:
Conducting comprehensive assessments to identify the medical, psychosocial, and practical needs of individuals with HIV/AIDS. Developing individualized care plans based on these assessments, outlining goals, objectives, and strategies for accessing and maintaining care.
- Service Coordination and Linkage:
Connecting clients with appropriate healthcare providers, including primary care physicians, specialists, and mental health professionals. Facilitating access to social services, such as housing, financial assistance, legal aid, and support groups.
- Treatment Adherence Counseling:
Providing education and support to help clients understand and adhere to their HIV treatment regimens. Addressing barriers to medication adherence and promoting consistent engagement in care.
- Advocacy and Support:
Serving as a client advocate, ensuring their needs and preferences are respected and addressed by healthcare providers and other service providers. Providing emotional support and encouragement to help clients navigate the challenges of living with HIV.
- Monitoring and Evaluation:
Tracking client progress, reviewing service plans regularly, and adjusting as needed to ensure optimal outcomes. Documenting all case management activities and maintaining accurate client records.
- Collaboration and Communication:
Working closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, social workers, and other specialists. Maintaining open communication with clients, their families, and other relevant stakeholders.
- Maintaining Expertise:
Staying up to date on the latest advancements in HIV treatment, care, and prevention. Understanding relevant policies, regulations, and funding streams related to HIV/AIDS services.
Essential Skills and Qualifications:
· Understanding of medical and psychosocial issues related to HIV disease and related conditions.
· Expertise in case management and client advocacy.
· Excellent verbal and written communication skills, active listening, and the ability to build rapport with clients from diverse backgrounds.
· A commitment to advocating for clients' needs and empowering them to make informed decisions about their health and well-being.
· Sensitivity to the diverse cultural backgrounds and experiences of individuals living with HIV/AIDS.
Education and Experience Requirements:
Bachelor’s degree in social work or other human service-related field (Addictions Counseling, Psychology, Sociology, Public Health, etc.)
Two years’ experience working as a Case Manager, Counselor, or related experience working in Human Services, Criminal Justice, Behavioral Health, or Public Health
