we are home health agency dedicated to delivering high-quality, compassionate home health care services across San Diego County. Our team of clinicians and aides work together to help patients regain independence, safety, and comfort at home.
We are seeking experienced and compassionate Certified Home Health Aides (CHHAs) to join our growing per diem team. The CHHA provides personal care and assistance to patients in their homes under the supervision of a Registered Nurse or Licensed Therapist, following individualized care plans.
Responsibilities
- Provide personal care including bathing, grooming, dressing, toileting, and mobility assistance
- Assist with basic exercises and ambulation as directed by the care plan
- Monitor and report changes in the patient’s condition to the supervising clinician
- Support patients with daily living activities and maintain a safe environment
- Document all care provided according to agency and regulatory standards
- Promote dignity, independence, and emotional well-being of each patient
