Work Environment:
- Fully Remote (WFH) – Must reside in HI, TX, IL, FL, or PA
- Must hold an active, in-good-standing pharmacist license in your state of residence
- Must have a dedicated, quiet, distraction-free workspace (HIPAA compliant)
- Must be able to hardwire into your modem (ethernet cord + surge protector required)
Schedule
- Regular Schedule: Monday–Friday, 8-hour shift starting between 9:00 AM – 11:00 AM CT
- Training: 8:00 AM – 4:30 PM CT (approx. 2 weeks)
Position Summary
The MTM Pharmacist provides telephonic Medication Therapy Management (MTM) to high-risk members. This role directly supports better health outcomes through medication reconciliation, identification of medication-related problems, and coordination of clinical interventions. Pharmacists communicate with patients, caregivers, prescribers, nurses, and internal teams to optimize medication regimens based on nationally recognized guidelines. Training includes motivational interviewing and behavioral coaching techniques to support member adherence and outcomes.
Key Responsibilities
- Conduct telephonic comprehensive medication reviews, including full medication reconciliation and clinical assessments.
- Identify, document, and resolve medication-related problems.
- Provide clear, health-literacy-appropriate written patient summaries.
- Prepare prescriber-facing fax materials to support clinical interventions.
- Build and maintain positive working relationships with patients, prescribers, nurses, and internal partners.
- Perform additional duties as assigned by leadership.
Required Qualifications
- Bachelor’s Degree in Pharmacy or PharmD
- Active pharmacist license (good standing) in HI, TX, IL, FL, or PA
- Verifiable High School Diploma or GED
- Ability to maintain productivity and focus in a remote environment
Preferred Qualifications
- Prior MTM experience (Outcomes MTM Connect highly preferred)
- Previous call center experience
- Strong organizational, communication, and interpersonal skills
- Proficiency with Microsoft Office and basic computer systems
- Experience meeting goals in a remote role
Department Summary
The Medication Therapy Management team supports vision of helping people reach better health outcomes. The MTM department identifies and resolves medication-related problems among high-risk members through coordinated care involving pharmacists, technicians, and support staff.
