    \n\t
  • Deliver skilled nursing care to patients in home health and hospice settings\n\t
  • Perform comprehensive assessments and develop individualized care plans\n\t
  • Administer medications, treatments, wound care, and pain management\n\t
  • Educate patients and families about disease processes, medications, and end-of-life care\n\t
  • Coordinate with physicians, case managers, and other interdisciplinary team members\n\t
  • Maintain accurate documentation using EMR systems\n\t
  • Manage a daily caseload and travel locally to patient homes\n

\nEducation & Certification Requirements:\n\n

    \n\t
  • Active RN License in the state of Idaho – required\n\t
  • BLS Certification (American Heart Association) – required\n\t
  • Valid driver's license and reliable transportation – required\n\t
  • ADN or BSN from an accredited nursing program\n

\nExperience Requirements:\n\n

    \n\t
  • Minimum of 1–2 years of recent experience in home health and/or hospice – required\n\t
  • Strong clinical assessment and organizational skills\n\t
  • EMR charting experience preferred\n\t
  • Previous travel nursing experience preferred but not required\n
